Registration begins immediately. A $100.00 deposit must accompany this form for registration and return it to the above address. We will make every attempt to place you into your first choice day of play. However, you may have to settle for your second choice. Make check or money order payable to "Cincinnati Sand Volleyball Club" or CSVC. A $25.00 fee will be charged for returned checks. League fees are nonrefundable unless we are not able to place you in your league. The league fee is nonrefundable balance must be paid in full prior to your first full game or a $25.00 late fee will be charged. Team captain is responsible for collecting and turning in all league fees. They also accept all financial responsibility for all league fees and returned check fees.
